• If you have already started entering data on the worksheet and encounter a problem, please contact Faith.Cirre@ky.gov  or Leah.Disponett@ky.gov .  Faith and Leah can be reached by telephone at (502) 573-1555. 
• If you have already submitted your budget/personnel worksheets, Leah will check the forms to verify that there is no error in the formula.  If any errors are found, Leah will correct the form and send you the updated version by close of business, May 18, 2006.
